What you should know about pre-Diabetes
Pre-diabetes means a person's blood glucose (sugar) level is higher than normal, but not high enough yet to be diagnosed as diabetes. If you have pre-diabetes, you are at increased risk for developing serious health problems such as type 2 diabetes, stroke and heart disease. The sooner people find out they have pre-diabetes and take action, the better their chances of preventing type-2 diabetes.

The Centers for Disease Control and Prevention (CDC) and the American Medical Association (AMA) are sounding an alarm about pre-diabetes because a national effort—by everyone from physicians to employers to patients to community organizations—is required to prevent type 2 diabetes in the United States. In addition to focusing on the person with pre-diabetes or diabetes, we also must engage the systems and communities where people live, work and play. We can all TEAM up to prevent diabetes.

How our TEAM Diabetes prevention program works
Prime Care Health Solutions offers the diabetes prevention program in a small group setting that is facilitated by a trained lifestyle coach that will help you build healthy lifestyle habits by introducing new options for healthy eating, physical activity and other healthy lifestyle changes.
All sessions are one-hour in length. The program begins with 16 consecutive weekly sessions, transitioning to three sessions every other week, then one session each month for 6 additional months to cement the positive changes to your new healthy lifestyle.
Topics covered include nutrition, getting started with physical activity, overcoming stress, staying motivated, and more.

If your test result is 5 or above, talk to your doctor about your risk for pre-diabetes. Your doctor may want to test your blood glucose levels to see if you qualify for the program.

Continued monthly sessions for the rest of the year after completion of a 16 week session. Sustained engagement with personal health coach and social network of support for entire year.
